The ProSpon silicone MCP/PIP joint replacement is a single-piece flexible implant for replacing the metacarpophalangeal and proximal interphalangeal joints of the fingers. It is supplied in seven sizes and used with a dedicated instrument set that includes trial templates for each size.

Indications

Replacement of the metacarpophalangeal (MCP) and proximal interphalangeal (PIP) joints:

  • fixed or stiff joints from progressive polyarthritis, or traumatic or arthritic destruction;
  • joint destruction or subluxation on X-ray;
  • contracture of the intrinsic muscles of the hand, and swan neck and boutonnière deformities;
  • ulnar deviation at the MCP joints that cannot be corrected by soft-tissue (ligament) reconstruction.

Contraindications

Infection, serious neuromuscular or vascular disease, insufficient bone or skin quality, and allergy to silicone. See the surgical technique for precautions.

How it works

The flexural zone sits at the centre of the implant, with a groove on the volar (palmar) surface for flexibility. Smooth conical stems of rectangular cross-section extend proximally and distally.

The stems sit loosely in the medullary canals and are not cemented. Their movement along the canal (the piston effect) allows flexion at the flexural zone, and the rectangular section resists rotation.

Instrument set

The implant is used with ProSpon's own instrument set:

  • handle with quick-release mechanism;
  • four-edged hand rasps for preparing the medullary canals;
  • an awl;
  • yellow silicone trial templates for the seven sizes, used to choose the size before implantation.

Sizes and ordering

W is the width of the flexural zone and L the overall length.
